RECOMMENDATION:
City staff are recommending City Council approval of Pay Application #2 (Final) and Change Order #2 for the Well #11
Directional Drill Project.

BACKGROUND/JUSTIFICATION:
The amount requested for Pay Application #2 includes payment for reestablishing vegetation, sod, sawing bituminous
pavement, and street patching. Change Order #2 in the amount of $3,504.57 includes payment for additional grading
and landscaping.
All required lien waivers, consent of surety and IC134 documents have been received.
